Build the athlete—skills, confidence, and a real plan.

We coach youth league athletes (fundamentals + confidence) and we focus heavily on middle school and high school development (skill, IQ, and recruiting readiness). This one form captures what Jamal needs to coach you correctly.

Student-Athlete + Youth Intake Form

Middle & high school athletes will complete most fields. Youth athletes can skip recruiting fields—staff will help tighten the profile later.

Parent / Guardian Info

We contact the parent/guardian first to confirm availability, location, and expectations.

Athlete Info

Core athlete details (youth through high school).

Youth leagues are welcome. Most families who complete every section are MS/HS.

Position & Style

This is what coaches always ask first: where you play and how you play.

This tells us how you *see* the game (style and role model).

Experience & Level

We use this to set the right intensity, expectations, and training plan.

Coach Jamal’s Questions

Real questions that tell us what to coach and how to coach it.

Training Timing & Expectations

Your “how soon” + “what do you want out of this” section.

Future Planning (Recruiting if applicable)

Youth can skip this. Middle/High school should answer if possible.

Athlete Agreements

Respectful culture. Coachable mindset. We’re building habits.

Parents: progress comes from consistency. We’ll give a plan—athletes have to follow it between sessions.

Parent/Guardian Consent

Clear acknowledgements (not legal advice).

Please allow a few business days after submitting—staff will contact the parent/guardian soon with next steps.

Note: Training is development support. Recruiting outcomes are never guaranteed.

Student-Athlete FAQ

Quick answers so athletes know what to expect and how to show up the right way.

What should I bring to training?

Basketball shoes, water, and a coachable attitude. If you have one, bring your ball (correct size). If you wear a brace, bring it.

What is a “good session” at ACE Unda Dawg?+

A good session means: effort, listening, reps with purpose, and correcting mistakes without quitting mentally.

Do I have to be “good” before I sign up?+

No. We coach development. If you’re willing to work and be consistent, we can build you.

What if I’m nervous or lack confidence?+

That’s normal. We train confidence through small wins, repetition, and game-real pressure reps.

How long until I see improvement?+

You’ll feel changes fast, but real results come from consistency. We’ll give you what to work on between sessions.

Parents FAQ

Clear expectations, safety, and how we measure progress—so you’re never guessing.

What ages do you coach?

We coach youth league athletes through high school. Our main volume is middle and high school, but youth athletes can absolutely train fundamentals when ready for structure.

How soon will someone contact me?+

Please allow a few business days after submitting. We review intakes in order and respond with scheduling and next steps.

What should we bring?+

Basketball shoes, water, and any braces needed. A ball is helpful but not required. For recruiting support, bring film links and academic info if available.

Are parents allowed to stay?+

Yes. Parents are welcome. Depending on the athlete, we may suggest less “sideline coaching” so the athlete can lock into instruction and build independence.

How do you track progress?+

We track improvements in fundamentals, decision-making, confidence, and game translation. Parents can request periodic updates on what’s improving and what’s next.

What safety measures are in place?+

Warmups, smart progression, hydration breaks, and age-appropriate intensity. We adjust for injuries/limitations and emphasize mechanics that reduce risk.

How do we get help fast?+

Email info@aceundadawg.com or call 816-281-8327. If you submitted the form, mention the athlete’s full name so we can locate the intake quickly.
